Registration packets must be turned in to the board members on 21 August between 6:00 and 8:00pm.

For those individuals who cannot make it to the rink on that day, there will be a "back up" registration date on 25 August from 6:00 to 8:00pm.

The following items must be turned in at one of these times. (Packets will only be collected if they are complete. Players who have not turned in their completed packets will not get on the ice for practices or games until the registration process is complete.)

Player Registration and Checklist
�� USA Hockey registration confirmation barcode for the 2008-2009 season (this registration must be completed on the usahockey website and printed)

�� Read USA Hockey Waiver of Liability Agreement and sign two copies

�� Complete and Sign USA Hockey Consent to Treat Form and Medical History Form

�� Player must read and sign USA Hockey Participant Code of Conduct

�� One Photocopy of Parents’ Health Insurance Card

�� One Photocopy of Players’ Birth Certificate

�� Read Parent/Legal Guardian Code of Conduct Agreement and sign

�� Check(s) (either a full payment or postdated checks as listed in the fee listing)
